Rules, Limitations & Exclusions
This plan offers discounted treatment at Advanced Dental & Implant Center. It is not an insurance plan and cannot be used at any other office.
- Membership lasts 12 consecutive months.
- Membership fees are paid in full at registration and are non-refundable once the plan begins.
- Benefits are non-transferable.
- A $50 fee applies to cleaning appointments missed or cancelled with less than 48 hours notice.
- Service fees are due at the time of booking unless otherwise arranged.
- A comprehensive exam is required before joining.
- Periodontal patients must enroll in the Periodontal Plan, not the Basic Plan. Basic Plan members who require periodontal treatment may complete their current plan year, then must switch to the Periodontal Plan at renewal.
- Plan benefits cannot be combined with other discounts and do not apply to office products (such as toothpaste, toothbrushes, and rinses) or orthodontic treatment.
- Rules, limitations, and exclusions are subject to change without notice.
